How Much Does Testosterone Replacement Therapy Cost?

The Short Answer

Testosterone replacement therapy (TRT) commonly runs anywhere from about $20 a month for generic injectable testosterone to $500 or more a month for gels, patches, or pellet implants, before lab work and provider visits are added in (Cleveland Clinic, n.d.). What a given patient actually pays depends heavily on the delivery method chosen, whether a diagnosed medical cause for low testosterone is documented, and whether Medicare or a private insurer will cover it. Out-of-pocket costs without any coverage can range from a few hundred to several thousand dollars a year.

What This Means

TRT is not one product with one price. It is a category of FDA-approved prescription formulations — injections, topical gels, transdermal patches, a buccal system applied to the gum, and subcutaneous pellets — each with its own manufacturing cost, dosing schedule, and insurance treatment (U.S. Food and Drug Administration [FDA], 2025). Generic injectable testosterone cypionate or enanthate is typically the least expensive option per month. Brand-name gels and patches cost more because they are patented and dosed daily. Pellets are placed by a clinician every three to six months and are billed as a procedure in addition to the cost of the pellets themselves.

Cost is also shaped by whether the therapy is billed through Medicare, a private commercial plan, or paid entirely out of pocket through a cash-pay clinic or telehealth platform. Coverage status changes the effective price more than any other single factor.

Candidacy

TRT is FDA-approved for men with confirmed low testosterone tied to a diagnosed medical condition affecting the testicles, pituitary gland, or brain — not simply for the gradual, age-related decline in testosterone that many men experience over time (FDA, 2025). The Endocrine Society's clinical practice guideline recommends diagnosing testosterone deficiency only in men who have both consistent symptoms (such as low libido, fatigue, or loss of muscle mass) and unequivocally low serum testosterone confirmed on repeat morning blood tests, not from symptoms or a single lab value alone (Bhasin et al., 2018). The American Urological Association's guideline sets a total testosterone level below 300 ng/dL, confirmed on two separate morning draws, as the threshold used to define low testosterone for treatment decisions (American Urological Association, n.d.).

This distinction matters for cost as much as for safety. Medicare and most private insurers will generally cover TRT when a documented medical cause of hypogonadism is present, but coverage is typically denied for testosterone prescribed for age-related decline or nonspecific fatigue with normal-range labs (Centers for Medicare & Medicaid Services [CMS], 2022). A candidacy evaluation is what determines which category a given patient falls into.

Clinical Evaluation

A proper work-up before starting TRT includes a review of symptoms, a physical exam, and testosterone levels measured on at least two separate mornings, since the hormone naturally peaks earlier in the day (Cleveland Clinic, n.d.). Additional labs typically screen for conditions that can mimic or contribute to low testosterone, and a baseline prostate-specific antigen (PSA) test is recommended for men over 40 before treatment begins, since testosterone therapy is not appropriate for men with untreated prostate cancer (American Urological Association, n.d.). This evaluation is billed separately from the medication itself and is usually covered as standard diagnostic testing, but patients paying cash for the whole process should expect it to add to the total.

Available Options

Patients diagnosed with a genuine testosterone deficiency have more than one path, and TRT is not the only one:

Generic injectable testosterone (cypionate or enanthate), self-administered every one to two weeks, is usually the least expensive formulation.

Topical gels and solutions, applied daily, cost more per month than injections and carry a risk of transferring testosterone to others through skin contact.

Transdermal patches are convenient but among the pricier daily options and can cause skin irritation.

Subcutaneous pellets, inserted by a clinician every three to six months, avoid daily dosing but involve a minor in-office procedure and a higher per-visit cost.

Non-hormonal alternatives — such as clomiphene citrate or lifestyle changes addressing obesity, sleep apnea, or medication side effects that can lower testosterone — may be appropriate instead of TRT for some men, particularly those who want to preserve fertility, since exogenous testosterone can suppress sperm production (Bhasin et al., 2018).

A licensed physician determines which of these fits a given patient's labs, symptoms, and goals; none of them is a one-size-fits-all default.

Benefits, Limitations, and Risks

For men with a confirmed deficiency, TRT can improve libido, energy, and muscle mass, though the size of the benefit varies by individual and is not guaranteed (Cleveland Clinic, n.d.). It is not a treatment for normal aging, and it will not reverse symptoms that are unrelated to low testosterone.

On safety, the evidence has shifted in recent years. In February 2025, the FDA removed the boxed warning about cardiovascular risk from all testosterone product labels after the large TRAVERSE trial found no increased rate of major cardiovascular events in men treated for hypogonadism compared with placebo (FDA, 2025). At the same time, the FDA required new or strengthened warnings about increased blood pressure across all testosterone products, based on separate post-marketing studies (FDA, 2025). Other known risks include an increase in red blood cell count (which requires monitoring), fluid retention, worsening sleep apnea, and reduced sperm production. Men with untreated prostate cancer should not start TRT, and those with a history of prostate cancer need individualized evaluation (American Urological Association, n.d.).

What the Evidence Shows

The evidence base establishes that TRT reliably raises testosterone levels and improves sexual function and some measures of mood and energy in men with confirmed hypogonadism, and that — per the TRAVERSE trial — it does not appear to raise the risk of heart attack, stroke, or cardiovascular death in this population over the trial's follow-up period (FDA, 2025). It also establishes a measurable increase in blood pressure as a class effect that requires monitoring.

The evidence does not establish that TRT slows aging, reverses age-related decline in men with normal testosterone levels, or produces benefits that outweigh its risks and costs for men without a documented medical cause of deficiency. Long-term cardiovascular data beyond the TRAVERSE trial's follow-up window, and long-term prostate safety data, remain areas of ongoing study rather than settled fact.

Next Steps and Monitoring

Once therapy starts, guidelines call for rechecking testosterone levels one to two months in, to confirm the dose is achieving a target in the normal range, followed by periodic monitoring of blood counts, blood pressure, and PSA (Bhasin et al., 2018). This ongoing monitoring is part of the total cost of care, not a one-time add-on, and is one reason a lower medication price alone does not tell the whole story of what a year of therapy will cost.

Patients considering a cash-pay or telehealth TRT program should ask directly whether the intake includes the repeat-morning-lab confirmation and baseline PSA screening these guidelines call for, since a program that skips that step is skipping the evaluation that determines whether TRT is appropriate in the first place.
